Session Staff FormsThe following lists the steps that are required to complete for your employment with the Senate Clerk’s Office.
Step 1- Download the forms Please download all forms and save them to your computer.
Parking Request (GRTC Riders will not need this form.) Please complete the forms below if you have not worked for the Senate in the past 3 years.
ID Badge Request Step 2- Print the forms The files that you have downloaded are stored on your computer. Most often, they are stored in the downloads folder on your computer. Find the location of the downloaded files and print each form. The forms are in Portable Document Format (PDF). To print the forms, you must have Adobe Acrobat Reader software installed on your computer (Adobe Acrobat Reader available at no cost). Step 3- Fill out forms Please read the forms carefully and complete the forms. Step 4– Scan the forms Scan and save the form individually. Use filenames that you can easily recognize, this will be important when submitting your forms. If you filled out 5 forms, after scanning the forms, you should now have 5 files. Important! -Make sure to scan the supporting documentation with the form that it supports. Step 5- Upload the forms Please complete the online information sheet by clicking the link below. After completing your information sheet the computer will show you a list of all the forms you need to upload. If you have to stop your submission, you will be able to return the forms submission screen later. Start Here.
